patients over 16 can now book appointments online. if you are not already registered for this service ask at reception. home visits and urgent appointments note – for emergencies dial 999 home visits visits are available for those who are housebound and are unable to leave their homes due their medical condition. for routine home visits please contact the practice before 10am or in an emergency, contact the practice and your request will be dealt with by a doctor at the earliest opportunity. urgent appointments if your appointment is urgent we will try to ensure that you are seen the same day. this may involve a return telephone call from one of the doctors – sometimes referred to as “triage” sick notes you only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. your employer will provide you with an sc2 self-certificate form for shorter periods of illness. test results please be sure to contact the practice to check your results such as blood tests, urine tests, x-rays and ecgs. receptionists can inform you of results (at the doctor’s discretion) but cannot discuss or interpret your results. to ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed. out of hours service an out of hours service is not provided by the practice. for an out of hours service, ring your practice telephone number and you will be directly connected to northern doctors or you can ring the northern doctors out of hours service direct on 0300 123 4343 www.northern-doctors.org local walk-in centre there is a primary care access centre based within wansbeck general hospital, ashington. this is a nurse-led centre that offers minor treatment services. repeat prescriptions you must ensure you allow 48 hours or 2 full working days when ordering a repeat prescription, during normal surgery opening hours. if ordering acute medication (items which require a doctor to authorise) please make sure you allow 72 hours or 3 full working days. requests can be made by telephone. blyth surgery has a prescription line which is open monday to friday 9.00 am to 4.00 pm. this number is: 01670 353586 prescriptions can also be ordered at reception or by sending a stamped addressed envelope. over 16?
